Forms

Before the Search

  1. Before beginning the search process, departments must fill out a Permission To Recruit (PTR) form.
    2. Attach advertisement and send to the Academic Personnel Office (APO).
    3. Advertisements may not be posted until approved by the Office for Faculty Equity.

During the Search

4. In order to comply with federal affirmative action regulations, demographic data must be collected for all candidates in all searches. Though it is required that all faculty searches utilize the Academic Search Data Collection Site, Non-Senate Academic searches may use the site or may collect the demographic data through mail in surveys.
  5. If opting to collect demographic data through mail in surveys, send letter to applicants requesting Candidate Demographic Data.

After the Search

6. Submit a List of Bona Fide Applicants and Candidate Demographic Data Form. This list and form can be generated automatically by the Academic Search Data Collection Site. If there are fewer than three applicants, the criteria for a targeted search must be met.
